  8.            Registering the Modem Doctor gives you access to one of the most
  9.          powerful diagnostic tools you can have for your PC. Registration is
  10.          simple.
  11.            Price for the Modem Doctor is $19.95 which includes US and Canada
  12.          shipping charges. Payment can be made by Check or Money Order. We
  13.          can handle COD's, however all shipping and handling charges for
  14.          COD's must be payed by you.
  15.            International Orders should submit a Money Order payable in US
  16.          funds plus $2.00 for shipping and Handling (except Canada where
  17.          shipping is included).
  18.            New York State residents should add the appropriate sales tax for
  19.          your county.

  21.          Remember...shipping is paid for in the US and Canada except for
  22.          COD's. International Orders must include $2.00. All payments in
  23.          US FUNDS.

  40.          an earlier version of the Modem Doctor, you can update to Version
  41.          3.0 for $5.00 + $2.00 for shipping.

  43.          your original disk or available when you first start the Modem
  44.          Doctor) with this form. Please fill out the serial number
  45.          information so we can send you the latest version of the Modem
  46.          Doctor.
