  9.     This declaration is made this __________ day of
  10. ____________________ 19________. I ___________________, being
  11. of sound mind, willfully and voluntarily make known my desires
  12. that my moment of death shall not be artificially postponed.

  15.     If at any time I should have an incurable and
  16. irreversible injury, disease, or illness judged to be a terminal
  17. condition by my attending physicians who has personally examined 
  18. me, and has determined that my death is imminent except for death
  19. delaying procedures, I direct that such  procedures which would
  20. serve only to prolong the dying process be withheld or withdrawn,
  21. and that I be permitted to die naturally with only the
  22. administration of medication, sustenance, or the performance of
  23. any medical procedure deemed necessary to provide me with comfort
  24. care.

  28. regarding the use of such death delaying procedures, it is
  29. my intention that this declaration shall be honored by my
  30. family and physician as the final expression of my legal right
  31. to refuse medical or surgical treatment and accept the
