  3. In order to receive a serial number to use with the PUTAV program, please 
  4. fill out and return this form to PKWARE.  When the enclosed information is 
  5. verified, you will receive your serial number from PKWARE.

  7. Please note that this does not constitute a license from PKWARE to 
  8. distribute the executable programs PKZIP, PKUNZIP or self-extracting PKSFX 
  9. files for commercial purposes.  See LICENSE.DOC or contact PKWARE Inc. for 
  10. more information about distribution licenses for the software.

  40. provide detection of tampered, hacked or virus infected files, we ask

  42. AV string.

  45. numbers, addresses, BBS numbers, BBS nodes or parameter, CompuServe
  46. addresses and similar information.
