Please be so kind as to forward to me one-sided copies of all medical records, medical bills, and x-ray records that you may have on behalf of our client. Please bill us for the costs incurred.
The records sent should include, among others: fact sheet, history and physical, progress notes, x-ray and laboratory reports, doctors' orders, temperature charts, operative reports, consultation reports and letters, discharge summary, medication administration records, emergency room records, operating room log, anesthesia log, incident reports, and investigative reports.
We appreciate your being certain that at least all of the above are included in the materials forwarded to us.
