SERSETUP version 1.2.5 PROBLEM REPORT

Please complete this information to the best of your ability. Do not
consider this confidential, but we will not use the information you
provide for purposes other than problem diagnosis and resolution. If
you can't answer some questions, just skip them - we'll come back to
them later if they're important.
It's usually easiest to use a text editor to complete this, then mail
