AMOS Bug Report Sheet

Thank you for taking the time to fill out this error sheet.
In order to correct errors in AMOS, I need to know about their existence.
This is possible only with your support.
Since I need very detailed information on your hardware configuration, as well
as a step-by-step guide on how to make the error occur, please use the
questionary below when reporting a bug.
The questions have been carefully selected, since I may be unable to reproduce
the error without the proper information.

If you have any special hardware installed, such as a network adapter,
sound-card or tape-drive controller, try unloading the drivers before
reporting the AMOS bug. If the error only occurs with the drivers
loaded, please give VERY detailed information about what driver causes
the problem, when it occurs and what hardware it is controlling.

What kind of error did you experience ? Please describe, as thorough
as possible, what led to the event, as well as a detailed description
of AMOS and OS/2 error messages resulting from the error.

How would you classify the error:
[ ] A - Fatal! AMOS is unusable and should be withdrawn until the
error is corrected. Significant data loss!
[ ] B - Fatal! One or more commands are dangerous in the extreme.
Should be corrected ASAP.
[ ] C - Bad. Beware when using some features; possible minor data
loss.
[ ] D - Irritating bug; does not result in data loss.
[ ] E - Insignificant bug, should be corrected in next release.
[ ] F - Purely cosmetical bug.
