  48. ALL  applications  MUST include payment of the required net fee BEFORE the
  49. application will be processed. Those paying by check or money  order  must
  50. mail the application and fee to:

  56. Checks and money orders should be made payable to DZ-Info Service, Inc.

  58. We  now  accept  Visa and MasterCard for payments. To use your credit card
  59. simply call the net host system (309-786-9823, 9624 or 9546). Applications
  60. must  be left as a comment to the SysOp only. From the Main command prompt
  61. type S (Script Questionnaire) and select the Net Fee option. Enter the
  62. appropriate data and your application will be processed in 24 to 48 hours!
  63.  
  64. For Canadian applicants only credit card charges or postal money orders (in
  65. U.S. dollars) will be accepted for payment.
  66.  
  67. Returned checks will result in a $17.50 fee.
  68.  
  69. In the event a system is declined for  membership  the  net  fee  will  be
  70. returned  within  5  business  days.  No  refunds  will  be given if false
  71. information is given on application.

  73. NOTICE: This application is ONLY valid for 60 days. Old  applications  and
  74. applications  submitted  without  proper payment are NOT processed. If you
  75. have an old application and wish to apply obtain  a  new  one  first,  the
  76. most  recent  Intelec Info Pack is ALWAYS available on the host system and
  77. is available on the first call.
