  4. form  to  be used to register the product.  It is  available
  5. under  the "Select Menu Item" dialog box by pressing "Create
  6. Order  Form".  A sample of the created order form is  below.
  7. Don't use it unless you can't create the order form by using
  8. the application.  See the Help file for further information.
