Files are saved with the long filenames for better precision. Please use the
MS Word format if you can, it will give much better results. A freeware viewer
is available on Microsoft's Internet site (www.microsoft.com).

To print the registration form from the MS Word document, simply print page 3
and 4. The results are far better than text. Please use print caracters.
