To receive your product, simply complete the order form below and mail it, along with your proof of purchase and payment, to the address listed at the end of the order form. This oOffer is good through March 31, 2001. Your oOrder must be postmarked by April 30, 2001.

Retail Customer Requirements:

To qualify for this offer �you must provide the following proof of purchase items for the corresponding Small Business Server 4.5 product:

Server:

� Submit a copy of a dated sales receipt for your server purchase (verify that it is legible and includes the purchase date, which must fall between January 1, 2000 and March 31, 2001); and

� Submit the complete original box top OR or original manual�s title page.

Client Add Pack (if applicable)):

� Submit a copy of dated sales receipt for any Client Add Pack purchase (verify that it is legible and includes the purchase date, which must fall between January 1, 2000 and March 31, 2001); and

� Submit the complete original bar code label from your Client Add Pack envelope. Or submit OR a photocopy of the label on your 3.5� Client Add Pack disk.

Open License Customer Requirements:

Open License Customers who acquire a license for BackOffice Small Business Server 4.5 between January 1, 2000 and March 31, 2001 are eligible to upgrade to Small Business Server 2000. �Contact the Worldwide Fulfillment Call Center at (800) /248-0655 and provide your Open Authorization Number to order your media.
