Patient's advance request for full information

I have drafted the following letter to my new GP to establish an understanding between us from the start. I would be interested to know how far others might find this a pattern for their own wishes - a sort of patients' charter:

'I am keen to remain independent of others and in control of my life as long as possible, and to that end to co-operate in my health care as fully as possible. I would like to have early diagnosis on any complaints, and full information about what symptoms may mean, of alternative treatments, and of risks taken in having or refusing them.'
