Do you mean at once you insert the disc or when you klick on its icon?
As far as I know you can set CacheCDFS to either use its internal player which
you can control via the cdda-icon or make it start an user-choosen cd-player. It can't both start the music and start the player program at the same time. So it's upto the player you use to handle any sort of "autoplay" option. I don't think "PlayCD" can do that.

I suggest you download "MiniCD" from Aminet:disk/cdrom and put it in your
WB-startup drawer. It's a very simple player, but also very useful and
powerful. You then only need one mouse-klick to start the music, or if you
want you can use AUTOPLAY=1 in the tooltypes. It will then play your cd at
once you insert it. If you then need more player control you just click on the
cdda-icon and the cd-player you have choosen from CacheCDFS will start, in
your case it's PlayCD. Both MiniCD and PlayCD happily work together as you will see...

Try it.. ..and use it! I do.. 8)
