This comment may not help but I did have a problem with scsi on my
wife's A4000 after the installation of AOS3.5 was made to work.
Here system froze during scsi writes fairly often, requiring a
reboot and thus invalidating the partition in use.

I'm not suggesting this is exactly your problem, but it opens up
another avenue of approach:

Her problem turned out to be a patch line in "devs:NSDPatch.cfg"
file which confused the version numbers. Hers was the incorrect
scsi device being patched, the lines were for the A4000T and her
scsi is an A4091.

Probably the best approach for installing AOS3.5 is to a clean
partition, installing the CD OS3.1 first and then updating that
to OS3.5. From there one can make any changes/additions desired.
