It's printed on the transporter keypad for you.  Restore to get it if needed.

There's a list of transporter locations in the repairwoman's pocket.  Take it.

Again, the codes are random.  Just use whatever you see on that sheet.  Some
of the sites have gender security systems on them, so you may have to change
back to a man to use them.
