Understand that filling out the Application for Eagle Enterprise Subscription
and billing Service is Subject to approval. I also understand that this can 
not and will not mean that I will be able to use this service. If for any 
reason that Eagle Enterprise finds my Application, BBS, or any other reason 
why I can not be accepted I understand that it is due to the policies set by 
Eagle Enterprise and nothing that I have done. I also understand that the 
Setup fee is to cover cost in processing my application and is NON-REFUNDABLE. 
I also understand that I may not charge my Application fee but only pay by 
check or money order only.

I have enclose my check or money order in the amount of 35.00 as my 
Application fee and understand also that this fee in no way will mean that I 
am accepted.
