      A user has attempted to access a file which  has previously been
      locked, used, and unlocked by the  application program.  However,
      the second time the file was  not locked.  Once locking is used on
      an opened file, it is  necessary for locking to be used
      consistently thereafter  when accessing the file.  This error
      message usually  indicates an application program bug!  The code
      has locked a  file in one place but neglected to lock the same
      file in  another.
